Are you a Respiratory Therapist who is passionate about improving resident outcomes, preventing avoidable hospitalizations, and making a lasting impact in long term care? We are seeking a Pulmonary Care Coordinator to lead and support our facility's pulmonary program through clinical excellence, interdisciplinary collaboration, education, and strong communication.

Pulmonary Care Coordinator Essential Responsibilities

• Complete pulmonary assessments and evaluations for residents enrolled in the pulmonary program
• Monitor respiratory status and identify changes in condition requiring intervention
• Develop, implement, and update individualized pulmonary care plans
• Coordinator respiratory interventions
• Serve as a primary pulmonary resource and communication leader for the facility
• Provide on-going education, coaching, and support to nursing staff, therapy staff, and respiratory personnel

Required Qualifications

• Current Registered Respiratory Therapist RRT license in good standing
• Current BLS certification

Preferred Qualifications

• Experience in skilled nursing, long-term care, rehabilitation, or post-acute care
• Leadership, supervisory, or program development experience
