Job summary

Qualifications

• Ambulatory clinic experience preferred; pulmonary/respiratory settings valued.
• Experience with telephone triage and EMR-based assessments.

Responsibilities

• Plan, implement and evaluate patient care for assigned clinic patients.
• Administer medications and monitor patient responses per policy.
• Collaborate with physicians and other health professionals to optimize care.
• Educate patients and families and develop discharge plans.
