The Public/Community Health Nurse (PHN) is responsible forthe mental and physical well-being of the community by ensuring the public’shealth through health promotion, disease prevention, treatment, andeducation. The Public/Community HealthNurse (PHN) can work in a variety of institutions such as the community,hospitals, clinics, or long-term care facilities while collaborating with stateregulators, doctors, and other healthcare professionals to keep the communitysafe and healthy.

Minimum Requirements:

• Active Nursing Licensure in the state practicing


• Two (2) years of public health or community experiencepreferred


• Complies with all relevant professional standards ofpractice


• Current CPR if applicable


• TB questionnaire, PPD or chest x-ray if applicable


• Current Health certificate (per contract or stateregulation)


• Must meet all federal, state and local requirements


• Must be at least 18 years of age
