Lucas County Health Department seeks a Public Health Nurse to manage pediatric/adolescent immunization programs, educate families, and coordinate off-site clinics under VFC/317 and Ohio guidelines. The role also oversees CMH client cases and supports Adult Travel, TB, and Reproductive Health nursing activities to improve health equity.

Formación

• ADN and 2 years of nursing experience
• Registered Nursing license in the state of Ohio
• Ability to utilize electronic medical software
• Ability to utilize MS Word and MS Excel
• Strong computer skills
• Knowledge of modern medical procedures and protocols
• Ability to work evenings & weekends during busy season
• Must have and maintain a valid driver’s license, reliable transportation, and be insurable

Responsabilidades

• Provides nursing care, clinical interventions, health screenings, case management, and program services including Adult Immunizations, Shots for Tots, TB, CMH, Reproductive Health, Travel Health Consultation
• Prepares and maintains client records in electronic health records systems
• Coordinates with community partners and agencies to promote population health and build coalitions
• Maintains vaccine inventory and monitors refrigerator temperatures; conducts off-site vaccination clinics
• Charts vaccinations and completes follow-ups; sends reminder postcards
• Audits charts to ensure best nursing practices
• Performs outreach, attends interagency meetings and collaborates with community agencies
• Performs initial client assessments and makes referrals
• Counsels clients and provides health education on parenting, child development, health and life cycle tasks
• Adheres to HIPAA and grant requirements; participates in quality improvement and PHAB competencies
• Mentors students as public health leaders and supports cross-training
