Job summary

Responsibilities include community health assessments, education and immunizations, maternal-child health support, and chronic disease prevention in collaboration with a supportive team.

Qualifications

• Bachelor's degree in nursing required.
• Preferred: Bachelor’s degree in nursing plus two years of experience.
• State of Wisconsin Registered Nurse License.
• Valid driver’s license.
• Experience with Communicable Disease control and prevention preferred.

Responsibilities

• Safeguards the community by assessing individuals and families through interviews, observations, and examinations to prevent the spread of communicable diseases.
• Provides nursing services and education to ensure access to clinical care, including pregnancy tests, immunizations, blood pressure checks, and referrals.
• Promotes maternal child and family health with services for pregnant women and families with young children, including breastfeeding support and developmental screening.
