Qualifications

Responsibilities

• Provides application assistance for federal, state, and local benefit programs.
• Facilitates connections to Harris County Public Health services and community resources.
• Conducts community outreach and engages residents at events and in various settings.
• Schedules appointments at HCPH Clinics to facilitate access to healthcare and preventive services.
• Documents client interactions and follow-up actions in designated systems with confidentiality.
