Job summary

NYC Department of Health and Mental Hygiene is seeking a Public Health Adviser to support clinics across NYC. The role involves reviewing patient lines, conducting disease intervention activities, and coordinating referrals to ensure high-quality care for New Yorkers.

Ideal candidates will have a health-related bachelor’s degree, experience in health promotion or disease intervention, and strong communication skills.

Qualifications

• Bachelor’s degree in health education or health, social or biological sciences with relevant credits.
• Experience in health promotion or disease intervention/prevention, including interviewing, referrals or data analysis.
• Ability to maintain confidentiality and follow protocols in public health settings.

Responsibilities

• Review clinic line lists to identify follow-up needs and referrals.
• Implement disease intervention and prevention activities.
• Coordinate referrals to ensure continuity of care.
• Conduct partner services interviews per public health guidelines and confidentiality.
• Assess records to determine follow-up actions and timely interventions.
• Assist with assignment of patients across clinics for workload balance.
• Monitor follow-up activities and communicate with staff on treatment and referrals.
• Provide technical assistance on disease intervention and documentation requirements.
• Maintain accurate health information system records per regulations.
• Collaborate with teams to improve access to prevention and treatment services.
• Provide onsite clinical support as needed (intake, counseling, waiting room).
• Participate in quality improvement meetings and training.
