Job summary

San Joaquin County Public Health Services is seeking a case manager to coordinate comprehensive care and provide counseling for clients with public health needs. The role requires a master’s degree (MSW or MFCC) and post-master’s experience, with strong assessment, outreach, and grant-writing capabilities.

You will collaborate with a multidisciplinary team, explain services to clients, and develop and adjust care plans while supporting advocacy and community programs within the county.

Qualifications

• Master’s degree in Social Work with supervised field placement.
• Or MFCC with supervised field placement meeting state standards.
• One year post-Master’s experience in counseling/case management for public health needs.

Responsibilities

• Explain program services and procedures to potential clients.
• Assess needs of new and continuing clients and plan services.
• Coordinate all aspects of case plans and modify as needed.
• Counsel clients and families; advocate with other agencies; provide crisis intervention.
• Serve as liaison between patients and health care team; recommend changes in procedures.
• Develop programs to meet community needs and assist with grant proposals.
