Job summary

The State of Washington's Department of Social and Health Services is seeking a PBS4 - Case Planning and Support Specialist to join the Virtual Case Management Center. You will assess eligibility, develop tailored case plans, and coordinate with healthcare, housing, and employment partners to connect clients with needed services.

Strong policy knowledge and compassionate communication are essential. Experience in public benefits and ability to handle complex cases in a trauma-informed approach

Qualifications

Must demonstrate deep knowledge of Washington public assistance programs.
Ability to read and apply WAC/RCW and internal policies.
Proven experience building service plans and coordinating with partners.

Responsibilities

Independently determines and re-determines client eligibility for CSD programs.
Conducts intensive client interviews and program orientations.
Develops tailored case plans and coordinates with healthcare, housing, employment services.
Verifies overpayments and processes referrals to Financial Recovery.
Monitors progress and updates service plans to remove participation barriers.
