Continuum Rehab Group in Rockwall, Texas is seeking a Licensed Physical Therapy Assistant to join our in-house therapy team serving Skilled Nursing/Long Term Care facilities. You will provide rehabilitative services under the supervision of a Physical Therapist and help patients regain function and independence.

Qualifications

• Licensed PTA with current license.
• Open to therapists at all experience levels.
• Provides rehabilitative treatment under supervision.

Responsibilities

• Implements evidence-based PT interventions.
• Treats patients with PT services per regulatory guidelines.
• Documents patient progress and communicates with supervising PT.
• Offers patient and family counseling and education.
• Assists in discharge planning and care coordination.
