Trilogy Health Services is seeking a Physical Therapist Assistant to join a senior living care team in Indiana. You will implement treatment plans created by Physical Therapists, observe patient responses, and document progress to guide ongoing therapy.

The role includes educating patients and families on exercises, safety, and home programs, while collaborating with an interdisciplinary team to ensure holistic patient care. Travel is minimal as needed.

Qualifications

• Provides therapeutic interventions under supervision of a Physical Therapist.
• Observes patient responses and documents progress.
• Keeps up-to-date documentation of patients’ responses and reports to the supervising PT.
• Educates patients and families on exercises and safety at home.
