Concept Rehab & Engage Consulting in Ohio is seeking a Physical Therapist Assistant (PTA) to join a therapist-owned organization delivering rehabilitation in SNF/post-acute settings. You’ll work under a Licensed PT, providing PTAs' treatments and collaborating with care teams.

New grads welcome; strong clinical judgment and communication are valued; the role includes documentation, education, and promoting patient independence.

Qualifications

• Active Physical Therapist Assistant (PTA) license in the state of practice.
• New graduates are encouraged to apply.
• Strong clinical judgment and communication skills.
• Commitment to patient-centered care and teamwork.
• Experience in Skilled Nursing Facility (SNF) or Post-Acute settings is a plus.

Responsibilities

• Provide physical therapy treatments according to established plans of care.
• Collaborate with Physical Therapists, interdisciplinary care teams, residents, and families.
• Maintain accurate and timely electronic documentation.
• Educate staff, residents, and family members on therapy programs and functional mobility strategies.
• Help residents achieve their highest level of independence and quality of life.
