Job summary

Our client operates a skilled nursing and long-term care facility in Ordway, Colorado, delivering rehabilitative and custodial care to elderly and post-acute patients. We seek a licensed PTA to join the rehabilitation team and support functional recovery under a supervising Physical Therapist.

The PTA will deliver individualized therapy, participate in care planning, document progress, and maintain a safe, compliant clinical environment.

Qualifications

• Associate degree from an accredited PTA program.
• Current, unrestricted PTA licensure in Colorado (or ability to obtain before start).
• Current CPR/BLS certification.
• Strong interpersonal and communication skills with a patient-first mindset.

Responsibilities

• Implement therapy plans under supervision of a licensed Physical Therapist.
• Deliver exercises, gait, balance, transfers, and modality interventions.
• Document progress and maintain compliant clinical records.
• Communicate with the PT, care team, and nursing staff about goals and progress.
• Educate patients and families on home exercise, fall prevention, and safe mobility.
