Our client is a trusted skilled nursing and long-term care facility serving residents in Ordway, Colorado and nearby rural communities. This PTA role supports rehabilitation for elderly and post-acute patients under licensed PT supervision, with a focus on dignity and quality of life.

The position emphasizes individualized therapy, documentation, and collaboration with an interdisciplinary team, offering relocation support and a supportive work environment in a stable facility.

Qualifications

• Associate's degree from an accredited PTA program.
• Colorado PTA licensure or ability to obtain before start date.
• Current CPR/BLS certification.
• Strong interpersonal and communication skills with a patient-first mindset.

Responsibilities

• Deliver individualized PTA services under supervision of a licensed Physical Therapist.
• Implement treatment plans including therapeutic exercise, gait training, and balance activities.
• Document patient progress and ensure compliance with regulatory standards.
• Collaborate with the interdisciplinary team and nursing staff on resident goals.
• Educate patients and families on home exercises and fall prevention.
• Maintain accurate clinical documentation in line with Medicare/Medicaid and facility requirements.
• Uphold resident rights, dignity, and confidentiality in all interactions.
