Job summary

South Haven Health & Rehab is seeking a Physical Therapist Assistant (PRN) to provide therapy to patients in a SNF setting under the supervision of a Physical Therapist. The role involves applying diverse therapeutic techniques, documenting progress, and participating as part of an interdisciplinary team to determine patient needs and plans of care.

New graduates are welcome for some locations, with emphasis on strong communication, organization, and CPR certification.

Qualifications

• Graduate of a Physical Therapy Assistant curriculum accredited by the APTA.
• Must hold a valid and current PTA license in the state employed.
• Demonstrates good verbal and written communication, and organization skills.
• Possesses and maintains current CPR certification.
• Must have a valid driver’s license with insured vehicle as required.

Responsibilities

• Provide therapy to patients under the supervision of a Physical Therapist in a SNF setting.
• Document patient progress clearly within designated time periods to show medical necessity of care.
• Utilize the computer system efficiently and adhere to company policy.
• Collaborate with an interdisciplinary team to determine patient needs and plans of care.
