Citrus Heights Post Acute in Citrus Heights, CA is seeking a licensed Physical Therapist Assistant (PTA) to join our rehabilitation team in a post-acute setting. Work under a licensed PT to deliver treatments and help residents regain mobility.

Qualifikationen

• Associate degree from an accredited Physical Therapist Assistant program.
• Experience in skilled nursing, post-acute care, rehabilitation, or long-term care preferred.
• Proficiency with electronic medical records and systems such as Net Health and PointClickCare preferred.

Aufgaben

• Provide physical therapy treatments as directed by the supervising Physical Therapist.
• Accurately complete all required documentation in accordance with facility, state, federal, and payer requirements.
• Maintain a safe treatment environment through adherence to infection control and safety standards.
• Participate in care plan meetings, interdisciplinary team meetings, and rehabilitation conferences as needed.
• Collaborate with nursing staff and assist with training restorative nursing aides.
• Communicate resident progress, goals, and discharge planning updates with the supervising therapist and interdisciplinary team.
• Educate residents, families, and caregivers regarding treatment plans, home exercise programs, safety awareness, mobility techniques, and fall prevention.
• Recommend and coordinate appropriate durable medical equipment including walkers, canes, splints, grab bars, and other assistive devices.
• Assist with quality assurance initiatives and rehabilitation program development.
• Provide facility in-service education and training as needed.
• Maintain confidentiality of resident and facility information.
• Build positive relationships with residents, families, staff members, and healthcare partners.
