Job summary

Rivertown Health and Rehab is seeking a compassionate Physical Therapy Assistant (PTA) to join our Therapy Team. You will provide PT-guided services to residents, support daily living activities, and help develop treatment plans to promote growth.

This per diem/PRN role offers opportunities for growth within a dedicated therapy department and a facility focused on resident outcomes and quality of care.

Qualifications

  • Associate’s degree in physical therapy from an accredited program.

Responsibilities

  • Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
  • Conduct screenings of residents at regular intervals to determine the need for intervention/treatment.
  • Assist in evaluations under the direction of the Physical Therapist.
  • Assist in identifying short- and long-term goals and treatment plans.
  • Conduct individual and group treatment as outlined in the plan, including strength, mobility, balance, gait, and ROM activities.
  • Assist in selecting therapy techniques/media and adapt as indicated by resident performance.
  • Monitor residents’ response to therapy and consult with the Physical Therapist for modifications.
  • Assist in scheduling in conjunction with the Physical Therapist and team members.
  • Adhere to department and resident schedules and modify as appropriate for treatment regimens.
  • Train residents, family and staff in equipment use.
  • Participate in resident, family, and staff education.
  • Contribute to the ongoing evaluation and documentation per CMS expectations in NetHealthEMR.

Status: Per Diem/PRN

Job Functions:

  • Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
  • Conduct screenings of residents at regular intervals todeterminethe needfor intervention/treatment.
  • Identifyindications for evaluations atappropriate intervalsandassistin the evaluation under the direction of the Physical Therapist.
  • Assist the Physical Therapist inidentifyinga target list, including short-term goals, long-termgoalsand treatment plan.
  • Conduct both individual and group treatment as assigned by the Physical Therapist and outlined in treatment plan that includes, but is not limited to, areas of muscle strength, coordination, mobility, sensory awareness, sitting and standing, transfer training, balance activities, gait training, range of motion activities and modalities.
  • Assistin theselectionof physical therapy techniques/media and in the sequence of activities, adapts techniques/media asindicatedby the resident’s performance and following consultation with the Physical Therapist
  • Monitor residents’ response to intervention andconsultwith the Physical Therapist if modification isindicated.
  • Assist in establishing and modifying individual resident schedules in conjunction with the Physical Therapist and other team members.
  • Adhere to department and resident schedule andmodifiesasappropriate fortreatment regimen.
  • Trainresidents, family andappropriate staffin the use or application of such equipment.
  • Actively participate in resident, family, and staff education.
  • Consistentlydemonstratesound judgment in the evaluation, planning, implementation, and follow-up of resident therapy programs.
  • Assistwith andparticipatesin Quality Assurance Program asrequired.
  • To complete compliant and medically necessary documentation from CMS expectations inNetHealthEMR systems

Requirements:

  • Associate’s degree in physical therapyfrom an accredited program.
  • Assumes responsibility for ongoing continuing education and professional development.
  • Must have an active Physical Therapy Assistant license in state(s) of practice.
  • Assume responsibility for ongoing continuing education and professional development tomaintaincurrent licensure and certification
