SUMMARY
The Physical Therapist Assistant (PTA) provides comprehensive skilled services under the direction of a Physical Therapist. The PTA educates patients and their family members and ensures safety of the patient. The PTA is accountable for completing accurate documentation and remains compliant with all legal rules and regulations. The Company adopts the physical therapy scope of practice as outlined by the state for its licensed PTA personnel.

QUALIFICATIONS & ATTRIBUTES
- Physical Therapist Assistant with a current license in the state of employment.
- Minimum of one year of experience as a PTA in an acute setting. Home care experience is desired.
- Effective communication and ability to create positive impressions with patients, families, physicians, and co‑workers.
- Knowledgeable on therapy best practices and able to make appropriate physical therapy judgments.
- Ability to identify situations and handle them with the best possible solution.
- Detail-oriented, observant, and disciplined with the ability to prioritize and be timely.
- Valid driver’s license and auto insurance with your name as a listed driver.
MAJOR AREAS OF RESPONSIBILITY
- Provide skilled services in a patient’s home as developed by the Physical Therapist plan of care.
- Observe, record, and report patient reactions, changes, and progress.
- Perform physical therapy procedures and diagnostic tests prescribed by a medical provider.
- Treat patients to relieve pain, develop or restore function, and maintain maximum performance using exercise, massage, heat, water, etc.
- Assess functional problems and provide intervention, working toward goals with the Physical Therapist.
- Communicate effectively with physicians, therapists, and coworkers.
- Participate in case conferencing to promote coordination of care.
- Recommend prosthetic/orthotic devices and make minor adjustments or recommend modifications.
- Educate staff and patients on physical therapy modalities, safety measures, and supportive activities.
- Instruct patients and families on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
- Listen to patient and family members and foster independence to prevent dependence on the agency.
- Complete all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines.
- Document all aspects of treatment, assessments, and patient education.
- Complete chart audits as requested and communicate any changes to scheduler.
- Participate in on-call requirements and case conferences, attend in-service trainings, mandatory agency meetings, and orientation/training of new employees.
- Perform other duties as assigned.
ESSENTIAL FUNCTIONS
- Drive to patient’s primary location per scheduled visit.
- Attend scheduled visits and document all aspects of visits within specified time frames.
- Follow the plan of care within the scope of practice for a licensed PTA.
PHYSICAL DEMANDS
Travel is required. Typical duties include reading, speaking, writing in English, communicating, computer/PDA usage, hand/finger dexterity, hearing, vision, walking, sitting, bending, standing, leading often to the use of equipment for lifts up to 50 lbs.
