Job summary

Origin Travel Nurses is seeking a Physical Therapist Assistant or Certified Occupational Therapy Assistant for home health services. This 13-week contract requires work Monday through Friday, 5 shifts of 8 hours each, with an expected 6–7 visits per day.

Documentation will be done in Homecare Homebase, with collaboration across disciplines to ensure quality patient care. Candidates must have home health experience, Oasis documentation proficiency, and active licensure as PTA or COTA.

• Experience in home health care is required.
• OASIS documentation experience is mandatory.
• Licensure as a Physical Therapist Assistant or Certified Occupational Therapy Assistant is required.

Responsibilities

• Conduct an average of 6-7 visits per day based on client type.
• Provide physical therapy and occupational therapy interventions as directed.
• Document patient progress and therapy outcomes using Homecare Homebase software.
• Collaborate with interdisciplinary teams to optimize patient care.
• Assist in covering additional territories within a 60-mile radius as needed.

Job Description

We are seeking a dedicated and skilled Physical Therapist Assistant or Certified Occupational Therapy Assistant for home health services. The ideal candidate will have experience with OASIS documentation and familiarity with Homecare Homebase. Candidates lacking HCHB experience may still be considered at a reduced rate for the initial training period, pending management approval.

Responsibilities

• Conduct an average of 6-7 visits per day based on the type of client visit.
• Provide physical therapy and occupational therapy interventions as directed.
• Document patient progress and therapy outcomes using Homecare Homebase software.
• Collaborate with interdisciplinary teams to optimize patient care.
• Assist in covering additional territories within the facility’s service area as needed, with a radius of approximately 60 miles.

Required Experience / Certifications / Licensure

• Experience in home health care required.
• Oasis documentation experience is mandatory.
• Licensure as a Physical Therapist Assistant or Certified Occupational Therapy Assistant is required.
