Oak Glen Healthcare and Rehabilitation Center in Kentucky is seeking a compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team. The PTA will promote, facilitate, and maintain health and participation in life under a physician-approved plan of care, ensuring safety and effectiveness of service delivery.

The role requires an Associate’s degree from a CAPTE-accredited program and an active state PT license, plus current CPR and at least 1 year of SNF experience.

Qualifications

• Associate’s degree from a CAPTE-accredited program.
• Active state Physical Therapy license in good standing.
• Current CPR certification.
• At least 1 year experience in a Skilled Nursing Facility preferred.
• Ability to manage multiple projects, tasks, and priorities.

Responsibilities

• Develop and implement individualized therapy programs to restore, reinforce, and enhance patient performance.
• Identify need for and refer to other healthcare disciplines as necessary.
• Instruct, educate, and train patients and caregivers in essential skills for independence and quality of life.
• Supervise PTAs, provisional licensees, students, and support personnel per licensure requirements and standards.
• Develop personalized care plans using various treatment techniques.
• Provide quality, personalized, evidence-based care.
• Promote healthy lifestyles by improving strength, flexibility, balance, and coordination.
• Continuously evaluate patient progress and communicate updates with the interdisciplinary team.
