Qualifications

• Associate's degree from a CAPTE-accredited program (required).
• Active state Physical Therapy license in good standing.
• Current CPR certification.
• At least 1 year of experience in a Skilled Nursing Facility (preferred).
• Strong interpersonal, communication, and leadership skills.

Responsibilities

• Develop and implement individualized therapy programs to restore, reinforce, and enhance patient performance.
• Identify needs and refer to other healthcare disciplines as necessary.
• Instruct, educate, and train patients and caregivers to promote independence and quality of life.
• Supervise assistants, provisional licensees, students, and support personnel in line with state licensure and standards.
• Develop personalized care plans using various treatment techniques.
• Provide evidence-based care using proven interventions.
• Promote healthy lifestyles by improving strength, flexibility, balance, and coordination.
• Monitor patient progress and communicate updates to the interdisciplinary team.
