Job summary

Oak Glen Healthcare and Rehabilitation Center in Kentucky is seeking a compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team. You will deliver patient-centered therapy under an approved plan of care and support safety and effectiveness of PTA services.

Responsibilities include developing individualized therapy programs, supervising aides, educating patients and families, and coordinating with the interdisciplinary team to promote independence and improved outcomes.

Qualifications

  • Associate's degree from a CAPTE-accredited program and active state license.
  • Current CPR certification.
  • At least 1 year experience in a Skilled Nursing Facility preferred.

Responsibilities

  • Develop and implement individualized therapy programs to restore, reinforce, and enhance patient performance.
  • Identify the need for and make referrals to other healthcare disciplines as necessary to address the comprehensive needs of the patient.
  • Instruct, educate, and train patients and caregivers in skills and functions essential for promoting independence and improving quality of life.
  • Supervise Physical Therapist Assistants, provisional licensees, students, and support personnel in accordance with state licensure requirements and professional standards.
  • Develop personalized care plans using a variety of treatment techniques.
  • Provide quality, personalized, and evidence-based care using proven interventions.
  • Promote clients’ healthy lifestyles by improving strength, flexibility, balance, and coordination.
  • Continuously evaluate patient progress, monitor outcomes, and communicate updates effectively with the interdisciplinary team

Summary of PhysicalTherapist Assistant position

The PTA is responsible for promoting, facilitating, and maintaining health, well-being, and participation in life.This individual is responsible for all aspects of physical therapy assistant service delivery provided under a physician approved plan of care and accountable for the safety and effectiveness of the physical therapy assistant service delivery process.

  • Supervise Physical Therapist Assistants, provisional licensees, students, and support personnel in accordance with state licensure requirements and professional standards.

  • Develop personalized care plans using a variety of treatment techniques.

  • Provide quality, personalized, and evidence-based care using proven interventions.

  • Promote clients’ healthy lifestyles by improving their strength, flexibility, balance, and coordination.

  • Continuously evaluate patient progress, monitor outcomes, and communicate updates effectively with the interdisciplinary team

Qualifications
Requirements of Physical Therapist Assistant (PTA):
  • Associate's degree from a CAPTE-accredited program Active state Physical Therapy license in good standing

  • Current CPR

  • At least one (1) year experience working in a Skilled Nursing Facility preferred

  • Ability to manage multiple projects, tasks, and priorities at once

  • Ability to maintain the confidentiality and integrity of the position

  • Excellent interpersonal, communication, and leadership skills

  • Ability to work independently and as part of a team

  • Commitment to providing high-quality, patient-centered care
