Job summary

Chautauqua Nursing & Rehabilitation Center in Dunkirk, NY is looking for a Physical Therapist Assistant to join our rehabilitation team. You will work under the supervision of a Physical Therapist to provide therapy that improves mobility, strength, balance and independence for residents.

This full-time position offers a Monday–Friday daytime schedule, competitive pay of $23.50 to $26.87 per hour, and benefits.

Qualifications

• Associate degree from an accredited PTA program.
• New York State PTA license required.
• Newly licensed PTAs encouraged to apply.
• Skilled nursing facility experience preferred.
• Strong communication, documentation and organizational skills.
• Ability to work collaboratively within an interdisciplinary healthcare team.

Responsibilities

• Provide skilled physical therapy treatments under the direction of a Physical Therapist.
• Assist with implementing individualized treatment plans designed to improve mobility, strength, balance, and functional independence.
• Monitor resident progress and communicate treatment updates to the supervising PT.
• Complete required documentation, treatment records, and care plan updates accurately and timely.
• Educate residents, families, and staff on therapy techniques, exercises, mobility, and adaptive equipment.
• Assist with discharge planning, home exercise programs, and equipment recommendations.
• Collaborate with physicians, nursing staff, and interdisciplinary team members to support resident outcomes.
• Participate in staff training, in-services, and quality improvement initiatives.
• Maintain therapy equipment, supplies, and treatment areas.
• Perform additional duties as assigned.
