Responsibilities

• Provide PTA services under the direct supervision of the Physical Therapist.
• Screen residents and determine intervention needs.
• Develop target goals and treatment plans with the Physical Therapist.
• Conduct individual and group therapy sessions per plan.
• Maintain CMS-compliant documentation in NetHealth EMR.
• Coordinate resident schedules with the PT and team.
• Educate residents, families, and staff on therapies.
