Job summary

Concho Health and Rehabilitation Center is seeking a Full-Time Physical Therapy Assistant (PTA) to join our in-house rehab team. You will provide person-centered care to help residents improve mobility, strength, and overall quality of life under PT supervision.

Qualifications

Active and valid Texas Physical Therapy Assistant license.
Current knowledge of PT treatment practices.
SNF or long-term care experience preferred.
Experience with Net Health documentation system preferred.

Responsibilities

Administer prescribed physical therapy treatments to improve function and mobility.
Provide skilled PT services under supervision of a Physical Therapist.
Respond to service requests and coordinate with the PT.
Modify plans of care under PT supervision.
Measure motor function, strength and muscle performance.
Monitor patient response and adjust approaches with the team.
Provide education to patients, families and caregivers.
Ensure timely and compliant documentation.
Attend required Rehab Director meetings.
