Job summary

Concourse Rehab & Nursing Center in Bronx, NY 10456 is seeking a Physical Therapy Assistant (PTA) to join our rehabilitation team. Working under a licensed Physical Therapist, you will assist residents through therapeutic exercises, mobility training, and restorative care in a patient-centered environment.

The role requires an Associate Degree from an accredited PTA program, NY PTA license, and BLS certification, with new grads welcome and experience in sub-acute rehab or long-term care

Qualifications

• Associate Degree from an accredited PTA program.
• Active, unrestricted New York State PTA license.
• BLS certification required.
• Sub-acute rehab or long-term care experience preferred.
• Strong communication, teamwork and documentation skills.

Responsibilities

• Assist in implementing individualized physical therapy treatment plans.
• Provide therapeutic exercises, gait training, and mobility activities.
• Monitor residents' responses and report progress to the Physical Therapist.
• Educate residents on exercises and mobility techniques.
• Maintain accurate and timely treatment documentation.
• Ensure therapy sessions comply with clinical and regulatory standards.
• Collaborate with the interdisciplinary care team to support resident outcomes.
