Job summary

Human Touch Home Health Care is seeking a dedicated Physical Therapist Assistant to join our Colorado team, delivering patient-centered home visits under PT supervision.

Qualifications

• Have completed a physical therapy assistant program accredited by the Commission on Accreditation in Physical Therapy or a comparable program.
• Currently licensed in the state(s) in which practicing, or eligible to take the PTA exam.
• Two (2) years' experience, preferred.
• Acceptance of philosophy and goals of Agency.
• Ability to exercise initiative and independent judgment.

Responsibilities

• Performs services planned, delegated and supervised by the Physical Therapist.
• Assists in preparing clinical and progress notes.
• Instructs patients and family in home programs and activities of daily living.
• Participates in in-service programs and presents as assigned.
• Maintains patient records and updates care plans per Agency policy.
• Communicates changes in patient condition to the PT and team.
