Job summary

ABL Health Care is seeking a compassionate Physical Therapist Assistant (PTA) to join our Home Health team in Rio Vista, CA. This position offers both Full-Time and Part-Time options, with the opportunity to collaborate with an experienced interdisciplinary team to deliver high-quality in-home care.

The PTA will evaluate patients, implement individualized treatment plans, document progress, and travel within a 15–25 mile radius as needed.

Qualifications

• California PTA license required.
• Home Health or Hospice care experience required (1 year).
• Valid CA driver’s license.
• Willingness to travel 15–25 miles regularly.

Responsibilities

• Provide PTA services within the patient’s plan in coordination with the Interdisciplinary Team.
• Administer treatments, monitor patient conditions, and report changes to the Physician, Case Manager, and Director of Nursing.
• Maintain accurate clinical records using a company-provided device.
• Comply with all company policies and procedures to ensure high standards of care.
