Job summary

PruittHealth @ Home seeks a Physical Therapist Assistant to provide home health PT services to patients in their residences in the Pilot Mountain, NC area. You will work under the supervision of a physical therapist, delivering care per the plan of care and collaborating with a multidisciplinary team.

Role requires an active PTA license in North Carolina, CPR cert, and at least two years of therapy experience.

Qualifications

• Must have a valid and unrestricted professional license in state of practice.
• Graduate of an accredited Physical Therapy Assistant program.
• If foreign-trained, the program must be deemed substantially equivalent by a credentials evaluation organization approved by the APTA.
• Current CPR certification.

Responsibilities

• Assist with the development and implementation of the plan of care within goals defined by the physician, patient/patient family and interdisciplinary team.
• Maintain continuity of care by collaborating with the interdisciplinary team and attending physician to develop, integrate, and manage the patient care plan of care.
• Demonstrate knowledge of home care and competency in discipline for specific patient care skills.
• Meet agency productivity standards and utilize time and resources effectively.
• Complete documentation timely, accurately, and at the point of care, according to industry standards.
• Assist the physical therapist in assessing the physical disability and level of function of patients.
• Maintain current license, certification, and clinical records to meet federal, state and agency regulations and guidelines.
• Participate in center/agency surveys (Licensure / JCAHO) and required reports.
• Report any alleged or suspected violations to supervisor until resolved.
• Be self-directed, prioritize, manage time, and meet outcomes and goals by deadlines.

Minimum of 2 years' therapy experience
