Job summary

This full-time position offers days shift and an hourly rate of $30 to $32.16, depending on experience, within a supportive, CMS-rated facility that emphasizes compassionate, resident-centered care.

Qualifications

• Associate degree from PTA program.
• New York PTA license required.
• Newly licensed PTAs encouraged to apply.
• Skilled nursing facility experience preferred.
• Strong communication, documentation, and organizational skills.

Responsibilities

• Provide skilled PTA services under supervision of a Physical Therapist.
• Implement individualized treatment plans to improve mobility, strength, balance, and functional independence.
• Monitor resident progress and communicate treatment updates to the supervising Physical Therapist.
• Complete required documentation, treatment records, and care plan updates accurately.
• Educate residents, families, and staff on therapy techniques, exercises, mobility, and adaptive equipment.
• Assist with discharge planning, home exercise programs, and equipment recommendations.
• Collaborate with physicians, nursing staff, and interdisciplinary team to support resident outcomes.
• Participate in staff training, in-services, and quality improvement initiatives.
• Maintain therapy equipment, supplies, and treatment areas.
