Job summary

Renewal Rehab is seeking a licensed Physical Therapist Assistant for per diem coverage in a SNF/long-term care setting in Illinois. You will provide therapy under supervision, select appropriate patients, and ensure timely evaluations and referrals with the PT team.

Strong communication and organization will help maintain compliant documentation and care quality. New grads are welcome; a mentorship program is available to support skill development.

Qualifications

• Current license as Physical Therapist Assistant in the state of practice.
• Associate degree from an APTA‑accredited PTA program.
• Strong communication and organizational skills.

Responsibilities

• Provide physical therapy treatments under the supervision of a licensed Physical Therapist in accordance with state practice guidelines.
• Assist in identifying appropriate patients for therapy and coordinate with the PT for evaluation and referral.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet regulatory requirements.
• Ensure all physician orders are obtained and up to date.
