Job summary

Qualifications

Responsibilities

• Implement treatment plans developed by the Physical Therapist.

• Maintain accurate documentation per policy and regulations.
• Educate patients on home exercise programs and safety.
• Collaborate with the rehabilitation team to optimize outcomes.
• Participate in care and discharge planning as needed.
