Job summary

Country Crest Post Acute is seeking a licensed Physical Therapist Assistant (PTA) to join our skilled nursing rehabilitation team in Oroville, CA. You will deliver evidence-informed physical therapy under a supervising PT and contribute to resident improvement and discharge planning.

Qualifications

• Active California PTA license required.
• Graduate of an accredited PTA program.
• Successful completion of NPTE for PTAs.
• Current CPR/BLS certification.

Responsibilities

• Implement resident treatment plans aligned with the plan of care.
• Deliver interventions targeting strength, mobility, balance, endurance, and functional performance.
• Monitor and report resident responses to the supervising PT.
• Educate residents and caregivers on home exercise programs and safety strategies.
• Complete accurate, timely clinical documentation per requirements.
• Maintain HIPAA, OSHA standards and patient rights compliance.
• Support discharge planning and transitions of care.
• Participate in team meetings and quality initiatives.
• Treat residents with dignity and professionalism.
