Tender Touch Rehab Services LLC is seeking a licensed Physical Therapist Assistant for a full-time role in a SNF/Long-Term Care setting in New Jersey. You will work under the supervision of a Physical Therapist to deliver high-quality therapy and coordinate patient care with the PT for evaluations and referrals.

Qualifications

• Associate’s degree from an APTA-accredited Physical Therapist Assistant program.
• Current license as Physical Therapist Assistant in the state of practice.
• Strong communication and organizational skills.

Responsibilities

• Provide physical therapy treatments under the supervision of a licensed Physical Therapist, following state practice guidelines.
• Assist in identifying appropriate patients for therapy and coordinate with the PT for evaluation and referral.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements; ensure all physician orders are current.
