Job summary

Renewal Rehab is seeking a licensed Physical Therapist Assistant for a full-time role in a SNF/Long-Term Care setting in Illinois. You will provide physical therapy treatments under the supervision of a licensed Physical Therapist and help identify appropriate patients for therapy while coordinating with the PT for evaluation and referral.

Qualifications

• Must be licensed Physical Therapist Assistant in state of practice.
• Associate’s degree from an APTA-accredited PTA program.
• Strong communication and organizational skills.

Responsibilities

• Provide physical therapy treatments under the supervision of a licensed Physical Therapist, following state practice guidelines.
• Assist in identifying appropriate patients for therapy and coordinate with the PT for evaluation and referral.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet regulatory requirements.
• Ensure all required physician orders are obtained and up to date.
