Renewal Rehab in Illinois is seeking a licensed Physical Therapist Assistant for a full-time position in SNF and long-term care settings. You will provide PT services under supervision, support patient evaluation, and coordinate with the supervising PT to ensure quality outcomes.

Qualifications

• Must be licensed Physical Therapist Assistant in the state of practice.
• Associate's degree from an APTA-accredited PTA program.

Responsabilités

• Provide physical therapy treatments under the supervision of a licensed Physical Therapist, following state practice guidelines.
• Assist in identifying appropriate patients for therapy and coordinate with the PT for evaluation and referral.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure all required physician orders are obtained and up to date.
