Job summary

Palms Care Center in Chowchilla, CA is seeking a compassionate Physical Therapist Assistant to join our therapy team. Under the supervision of a licensed Physical Therapist, you will help residents regain strength, mobility, balance, and independence.

The PTA will carry out treatment plans, provide therapeutic exercises, monitor progress, document outcomes, and collaborate with nursing staff and physicians to support residents' rehabilitation goals.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current California PTA license in good standing.
• Skilled nursing facility experience preferred but not required.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a team-centered environment.
• Passion for helping others achieve independence.

Responsibilities

• Carry out treatment plans established by the Physical Therapist
• Provide therapeutic exercises, gait training, balance activities, and mobility interventions
• Monitor resident progress and communicate findings to the Physical Therapist
• Accurately document treatments, resident responses, and progress toward goals
• Collaborate with nursing staff, therapists, physicians, and interdisciplinary team members
• Educate residents and families regarding therapy programs and safety recommendations
• Maintain compliance with all facility, state, federal, and professional standards
