Job summary

Blue Central Group is seeking a dedicated Physical Therapist Assistant (PTA) to join our skilled nursing facility team in Naples, FL. You will work closely with licensed physical therapists to deliver patient-centered care and help restore mobility.

You will perform therapeutic exercises, manual therapy, and use equipment, while documenting progress in the EHR and supporting discharge planning across inpatient, outpatient, and home health settings.

Qualifications

• Ability to assist in developing care plans for geriatrics, orthopedics, and post-surgical cases.
• Maintain accurate clinical records and documentation.
• Knowledge of discharge planning and care transitions.
• Familiarity with EHR documentation and clinical terminology.

Responsibilities

• Assist licensed PTs in developing and implementing individualized care plans for geriatrics, orthopedics, and post-surgical cases.
• Perform therapeutic exercises and manual therapy to promote healing and mobility.
• Operate medical equipment safely during therapy sessions.
• Document patient progress in electronic health records (EHR).
• Support discharge planning and adjust treatment as needed.
• Collaborate with interdisciplinary teams across multiple settings.
• Contribute to patient care in SNF, outpatient, inpatient, home health, and acute care environments.
