Qualifications

• Assist licensed physical therapists in geriatrics, orthopedics, and post-surgical cases.
• Develop and implement individualized care plans with therapists.
• Document patient progress using clinical terminology and standards.

Responsibilities

• Assist licensed physical therapists in developing and implementing individualized care plans for geriatrics, orthopedics, and post-surgical cases.
• Perform therapeutic exercises, manual therapy techniques such as trigger point therapy, and modalities to promote healing and mobility.
• Operate medical equipment safely and effectively during therapy sessions, ensuring proper usage and maintenance.
• Document patient progress accurately in accordance with clinical documentation standards and medical terminology guidelines.
• Conduct thorough clinical assessments and record findings in electronic health records (EHR), supporting effective discharge planning and ongoing treatment adjustments.
• Support patient care activities across multiple settings including outpatient clinics, inpatient units, skilled nursing facilities, home health visits, and acute care environments.
• Collaborate with interdisciplinary teams to develop comprehensive care plans that address individual patient needs and goals.
