Job summary

Preferred Care Home Health in Naples, FL is seeking a full-time Physical Therapist Assistant (PTA) to provide skilled home health therapy under a licensed PT.

You will guide exercises, document progress, and collaborate with physicians, nurses, and caregivers to support mobility and function in patients' homes.

Qualifications

• Graduate from an accredited PTA program.
• Active PTA license in the state of practice.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Home health, rehab, or skilled nursing experience is a plus.
• Strong communication, organizational, and documentation skills.

Responsibilities

• Deliver skilled physical therapy under the direction of a licensed PT and in line with the physician's plan of care.
• Guide patients through therapeutic exercises to build mobility, strength, balance, and function.
• Track and document patient progress toward treatment goals.
• Teach patients and caregivers safe techniques and home exercise programs.
• Complete clinical notes and progress reports accurately and on time.
• Collaborate with physicians, therapists, nurses, and caregivers as part of a coordinated care team.
• Take part in case conferences and care planning as needed.
