Job summary

Napa Valley Care Center in Napa, CA is seeking a motivated Physical Therapist Assistant (PTA) to join our rehabilitation team. You will work closely with Physical Therapists to help patients improve mobility, restore function, reduce pain, and achieve independence.

This is an excellent opportunity for a dedicated PTA who values patient-centered care in a skilled nursing and rehabilitation setting. Full-Time with flexible scheduling is offered, with competitive hourly pay and a collaborative team

Qualifications

Responsibilities

• Implement treatment plans developed by the Physical Therapist.
• Provide therapeutic exercises to improve strength, mobility, balance, and endurance.
• Assist gait training, transfers, mobility activities, and use of adaptive equipment.
• Monitor patient responses and report progress to the Physical Therapist.
• Maintain accurate documentation per facility and regulatory standards.
• Educate patients and families on home exercises and safety techniques.
• Collaborate with interdisciplinary team to optimize outcomes.
• Participate in care planning meetings and discharge planning as needed.
• Maintain a safe, supportive, and motivating treatment environment.

Napa Valley Care Center is seeking a motivated and compassionate Physical Therapist Assistant (PTA) to join our dynamic rehabilitation team. In this role, you will work closely with Physical Therapists to help patients improve mobility, restore function, reduce pain, and achieve their highest level of independence.

What You'll Do

As a Physical Therapist Assistant, you will:

• Implement treatment plans developed by the Physical Therapist
• Provide therapeutic exercises and interventions to improve strength, mobility, balance, endurance, and functional performance
• Assist patients with gait training, transfer training, mobility activities, and use of adaptive equipment
• Monitor patient responses to treatment and communicate progress to the Physical Therapist
• Maintain timely and accurate documentation in accordance with facility, state, and federal requirements
• Educate patients and families on home exercise programs, safety precautions, and mobility techniques
• Collaborate with interdisciplinary team members to optimize patient outcomes
• Participate in care planning meetings and discharge planning as needed
• Ensure a safe, supportive, and motivating treatment environment

Qualifications

Required

• Graduate of an accredited Physical Therapist Assistant program
• Current California Physical Therapist Assistant (PTA) license in good standing
• Current CPR certification (or ability to obtain upon hire)
• Excellent communication and interpersonal skills
• Ability to work effectively in a team environment

Preferred

• Experience in a skilled nursing facility, post-acute care, rehabilitation, or long-term care setting
• Knowledge of electronic medical records and therapy documentation systems
• Strong understanding of Medicare, managed care, and rehabilitation practices
