Job summary

The Christ Hospital Home Health Care is seeking a Physical Therapist Assistant I to provide skilled services in patients' homes under the supervision of a Physical Therapist. The role includes documentation, patient education, and coordination with the care team in Greater Cincinnati and surrounding Southwest Ohio communities.

You will educate patients and families on therapy techniques, assist with devices, and participate in case conferences.

Qualifications

• One year of PTA experience in an acute setting is required.
• Home care experience is desired.
• Ability to effectively communicate with patients, families, physicians, and coworkers.
• Valid driver’s license and auto insurance with your name as a listed driver.

Responsibilities

• Provide skilled PTA services in patients' homes as prescribed by the plan of care.
• Observe, document, and report patient status and progress.
• Perform PT procedures to relieve pain and restore function.
• Educate patients and families on therapy techniques and safety.
• Complete clinical documentation per Medicare/Federal guidelines.
• Coordinate with physicians and the care team; participate in case conferences.

The Physical Therapist Assistant educates patients and their family members and ensures safety of the patient. The PTA I is accountable for completing accurate documentation and remains compliant with all legal rule and regulations.
