Job summary

Freedom at Home in Michigan is seeking a compassionate Occupational Therapist to join our home health team. Evaluate patients in their homes, develop individualized treatment plans, and help patients regain independence while delivering high-quality, patient-centered care.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Michigan OT license.
• Current CPR certification.
• Minimum of 2 years OT experience; home health preferred.

Responsibilities

• Evaluate patients in their homes and develop individualized treatment plans.
• Provide skilled OT services to improve independence and quality of life.
• Assess home safety and recommend adaptive equipment or modifications.
• Educate patients and caregivers on exercises, self-care techniques, and safe use of assistive devices.
• Complete OASIS assessments and clinical documentation accurately and on time.
• Collaborate with physicians, nurses, and other healthcare professionals to achieve the best patient outcomes.
• Monitor patient progress and adjust treatment plans as appropriate.
• Promote safe, compassionate, high-quality patient care.
