Job summary
Forever Staffing Healthcare Agency is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team in California. This full-time position focuses on evaluating residents, creating individualized plans, and delivering interventions to improve mobility and independence in a skilled nursing/long-term care setting.
You will work with patients, families, nursing and rehab staff to ensure high-quality care and accurate documentation in EMR systems.
Qualifications
- Graduate of an approved/accredited Physical Therapy program.
- Current, active Physical Therapist license in state of practice.
- CPR certification as required.
- Strong communication, documentation, organizational and customer-service skills.
- Ability to work effectively with patients, families and interdisciplinary teams.
Responsibilities
- Evaluate patients and develop individualized treatment plans.
- Provide skilled PT services to maximize mobility and independence.
- Monitor progress and adjust plans as needed.
- Complete evaluations, daily notes, summaries, and monthly reports.
- Document patient information accurately in EMR and rehab systems.
- Educate patients, families and staff on treatment plans and outcomes.
