Job summary

Stern Therapy Consultants is seeking a PRN Physical Therapist to provide coverage at Autumn Lake at Windsor, a skilled nursing facility in Windsor, CT. The PT will evaluate residents, develop treatment plans, and collaborate with the care team to promote safer mobility.

This PRN role offers hourly pay of $55–$60 with opportunities to contribute to residents' rehab goals and participate in a supportive rehab team led by regional leadership.

Qualifications

  • Active Connecticut Physical Therapist license.
  • Strong clinical judgment and documentation skills.
  • Skilled nursing experience is a plus.

  • Develop treatment plans focused on strength, balance, walking, and transfers.

Physical Therapist (PT) - PRN

Autumn Lake at Windsor | Windsor, CT

$55-$60/hour | PRN

Stern Therapy Consultants is seeking a PRN Physical Therapist to provide coverage at Autumn Lake at Windsor. You'll help residents in a skilled nursing setting work toward safer mobility through evaluations, individualized treatment, and close coordination with the care team.

What You'll Do
  • Evaluate residents after a hospital stay or change in mobility
  • Develop treatment plans focused on strength, balance, walking, and transfers
  • Provide therapy and reassess progress as residents' needs change
  • Communicate mobility recommendations to nursing and other clinicians
  • Complete evaluations, progress notes, and treatment documentation
