Responsibilities

• Perform evaluations that consider residents' movement abilities, medical precautions, prior function, and personal goals.
• Establish individualized plans of care with measurable targets for mobility and physical performance.
• Deliver gait training, therapeutic exercise, balance activities, and transfer training.
• Determine appropriate mobility equipment and instruct residents and caregivers in its safe use.
• Reevaluate progress and provide clinical direction to PTAs involved in treatment.
• Complete accurate documentation and work with nursing and fellow therapists on care coordination and discharge planning.

Stern Therapy Consultants is looking for a PRN Physical Therapist to evaluate and treat adults in a skilled nursing rehabilitation setting, helping them build the mobility needed for daily activities and their next stage of care.

What You'll Do

• Perform evaluations that consider residents' movement abilities, medical precautions, prior function, and personal goals.
