Job summary

Stern Therapy Consultants is seeking a PRN Physical Therapist (PT) for Langdon Place of Keene, NH. This role offers a flexible schedule, allowing you to help residents maximize their mobility and independence in a skilled nursing and post-acute care setting.

Qualifications

• Active New Hampshire Physical Therapist license required.
• Strong communication and clinical reasoning skills.
• Skilled nursing experience preferred but not required.

Responsibilities

• Complete physical therapy evaluations and develop individualized treatment plans.
• Provide treatment focused on gait, balance, strengthening, transfers, mobility, endurance, and pain management.
• Monitor resident progress and modify treatment plans as appropriate.
• Collaborate with PTAs, OTs, SLPs, nursing staff, and physicians to coordinate resident care.
• Educate residents and caregivers on exercises and safety techniques.
• Complete accurate and timely documentation.

We are seeking a PRN Physical Therapist (PT) to support our rehabilitation team. This role offers flexible scheduling while providing the opportunity to help residents maximize mobility, strength, and independence in a skilled nursing and post-acute care setting.
