Autumn Lake at Memorial Bridge in Carneys Point, NJ is hiring a PRN Physical Therapist through Stern Therapy Consultants. Bring your clinical judgment and hands-on treatment skills to help skilled nursing residents improve functional mobility and meet the demands of everyday life.

This PRN role offers $60–$65 per hour, opportunities based on facility coverage, and ongoing support from therapy leadership and rehabilitation colleagues.

Qualifications

• Active New Jersey Physical Therapist license.
• Strong assessment, treatment planning, and clinical reasoning skills.
• Effective communication and timely, accurate documentation.
• Skilled nursing experience is helpful; new graduates are welcome to apply.

Responsibilities

• Perform evaluations that identify movement limitations, rehabilitation potential, and assistance needs.
• Develop individualized plans with measurable goals for strength, balance, walking, and transfers.
• Deliver treatment that reflects residents' medical precautions, current abilities, and response to activity.
• Recommend mobility devices and train residents and caregivers in safe movement techniques.
• Reevaluate outcomes, update treatment plans, and supervise PTA participation in care.
• Maintain clinical documentation and collaborate with nursing and other therapists on mobility guidance and discharge recommendations.
