Stern Therapy Consultants is hiring a PRN Physical Therapist at Bedford Hills Center in Bedford, NH. Join a skilled nursing rehabilitation team and help residents improve mobility through treatments grounded in their health needs and recovery goals.

You will evaluate residents, develop goals, provide therapeutic exercises and gait training, assess assistive devices, supervise PTA services, and maintain documentation while collaborating with nursing and rehab colleagues on discharge planning.

Qualifications

• Active NH Physical Therapist license required.
• Strong evaluation, treatment planning, and clinical reasoning skills.
• Clear communication and accurate documentation.
• Experience in skilled nursing or post-acute rehab preferred; new grads welcome.

Responsibilities

• Evaluate residents' strength, balance, ROM, and mobility.
• Develop individualized care plans with measurable goals for walking, transfers, and independence.
• Provide therapeutic exercise, gait training, and mobility interventions.
• Assess assistive device needs and educate residents and caregivers on safe movement strategies.
• Reassess function, adjust treatment, and supervise PTA services.
• Maintain timely clinical documentation and collaborate on discharge planning.

What You'll Do

• Evaluate residents' strength, balance, range of motion, and ability to manage everyday movement.
• Develop individualized plans of care with measurable goals for walking, transfers, and functional independence.
• Provide therapeutic exercise, gait training, and mobility interventions suited to each resident's condition.
• Assess assistive device needs and educate residents and caregivers on safe movement strategies.
• Reassess function, adjust treatment approaches, and supervise PTA services.
• Maintain timely clinical documentation and collaborate with nursing and rehabilitation colleagues on care recommendations and discharge planning.
