Description
- Evaluate patients within twenty-four (24) hours or one (1) business day of physician referral. If unable to complete evaluation in this timeframe, document the cause for delay clearly in the medical record.
- Develop effective treatment plans and obtain approval for services from the referring physician.
- Treat patients according to the physician treatment plan.
- Assist the nursing department with training Restorative Aides.
- Supervise Physical Therapy Assistants in direct patient care and related activities, following state practice acts.
- Communicate with supervisors and other healthcare team members regarding patient progress, problems, and plans.
- Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed.
- Conduct in-service training for staff within the facility.
- Document evaluations, daily treatment notes, weekly progress notes, recertification documentation, in accordance with PT Board State Practice Act and payer requirements.
- Record treatment modifications per policies and procedures.
- Instruct patients' families or nursing staff in maintenance programs and caregiver training in preparation for discharge from therapy services.
- Participate in discharge planning.
- Recommend and facilitate the ordering of necessary durable medical equipment, such as braces, walkers, canes, elevated toilet seats, and grab bars, to promote patient independence.
- Report any equipment issues to ensure proper maintenance.
- Maintain positive, effective, and appropriate relationships with patients/residents, families, staff, and colleagues.
- Supervisory Requirements: Supervise PT assistants, aides, and students.
- Qualifications: Bachelor's degree in Physical Therapy; Master's or Doctorate preferred. Proficient in computer skills (e.g., Casamba, Point Click Care). Ability to read technical procedures, policies, and respond effectively to questions. Must hold a valid state license and meet continuing education requirements. Physical ability to perform job functions as described.
- Work Environment: Usually low to moderate noise level. Physical demands include frequent standing, walking, reaching, talking, and smelling; occasional lifting of over 100 pounds.
